The First Turn of a Bolt Must Never Be the Pressure Test
A pipe, hose, filter or vessel can retain pressure and hazardous contents after the normal process has been stopped and a gauge appears clear. The first movement of a bolt, clamp or fitting must never be used to discover whether the isolation worked. Safe opening depends on positive identification, controlled isolation and an independent means of proving the equipment is depressurised, drained and suitable to open before anyone enters the line of fire.
Controls to confirm
- Positively identify the exact equipment and contents, review the approved line-opening plan and permit, and confirm that the task, boundaries, simultaneous operations and responsible persons match conditions in the field.
- Isolate, lock or secure, depressurise, drain, vent, purge or flush as required, then prove the condition through an approved bleed, drain or test point while accounting for blocked passages, trapped sections and thermal pressure.
- Wear the specified protective equipment, establish an exclusion area and position the body away from the release path; open gradually using the approved method and stop immediately if pressure, temperature, odour, liquid or unexpected resistance is detected.
Today’s action
Before the first process opening today, trace the isolation boundary in the field and ask the work party to show the independent point that proves the system is safe to open.
